Introduction
Welcome to the Athens Sports Medicine home page. Here you will find contact information for our staff athletic trainers, information regarding physicals, & how to complete the yearly consent forms required by the UIL and Athens ISD. RankOne Sport is the mandatory format that all yearly consent forms will be completed. There is no need to create an account, but you will need your student’s ID number. There is a page for Emergency Contact information and a page for UIL Consent forms. Both of these pages are required for sports participation & must be filled out completely before the athlete will be cleared to play. This is in addition to their athletic physical that is required before the athlete can practice or play in games.

Athletic Physicals
All incoming 2021-22 7th grade student athletes & anyone else who has never received a physical for sports participation will be required to get a physical from a MD, NP or PA. This is a more thorough physical exam and is mandatory for these athletes. We will conduct mass physicals at the high school fieldhouse in the spring of 2022 for students who already have a physical on file. Athletic physicals are valid for two(2) years after they are completed and turned into the Athletic Trainer.

UIL and Athens ISD Consent Forms
The consent forms required by Athens ISD and the UIL are all completed online through RankOne Sport. The link to the page to complete these forms is located on the hyperlink above. These must be completed every year prior to your students participation in any school related athletic practices or games.
